Getting Started With Generation Care
Where you go for therapy services is always your choice whether your physician refers you or you seek out therapy on your own. Effective on January 1st, 2015, Michigan law allows consumers in Michigan to seek and receive physical therapy services WITHOUT a physician order. Most patients don’t realize that a referral to therapy means they can see whomever they choose and we hope that you will chose Generation Care. If you are interested in learning how Generation Care may help you with your health or wellness, there are several access options:
Call and schedule an appointment with one of our therapists. If you would like to utilize your health benefits and your insurance carrier requires a physician’s order we will contact them for you. We also have affordable self-pay rates if you do not want to involve your insurance carrier.
Make an appointment with your physician. We are happy to provide any materials that may help explain our services to you or your physician – just call and ask us! We’re happy to help.
Contact your physician for a referral. If your physician is aware of your problem, you may simply need to ask for a referral to us. Your physician may wish to see you personally prior to determining whether he or she feels a therapy referral is appropriate.

Physician Referrals + Downloadable Forms
Let your doctor know you want to come to Generation Care for your care. We work with ALL of the local physicians as well as doctors from University of Michigan, Mayo Clinic, Cleveland Clinic, and anyone out of state.
Once a referral is made to Generation Care, either by you or your physician, we will immediately set your initial appointment at a convenient time for you. If transportation is an issue, we can help assemble resources for you.
- If your appointment is the next day, you can either complete the pre-registration materials by downloading them (see Pre-Register) and bringing them to your first appointment. Or come 10-15 minutes prior to your appointment to complete your rehabilitation history form.
- Bring any relevant information – surgical reports, physician prescriptions, etc. – anything that you feel would be helpful to your therapist.
- If your referral is specific to aquatic therapy, you will likely have a land evaluation first.
- Bring your insurance card(s), identification card, physician’s prescription, if one was given to you, and list of medications you are currently taking.
- If you would like reports sent to additional people other than your physician, please bring names and addresses of case managers, etc.
- Wear comfortable clothing relevant to your condition. For example, if you have a knee injury, either wear clothes that will allow us to view and touch your leg or bring shorts.
Your first appointment will include an evaluation and the beginning to your treatment. We’ll be forming a plan of care that works for you and scheduling additional appointments as needed.
If your care includes home exercises or other instructions, you will receive a folder for ongoing use and gathering of materials.

If you have specific questions about coverage, please feel free to contact us and we will help you determine if prior authorization is needed, your remaining deductible, co-insurance and co-pay information. Every program varies greatly and therefore we will do our best to have the correct benefit information for you however we always advise that our patients be informed of their own benefits as well.

Your clear understanding of our financial policy is important to our professional relationship. We must emphasize that as a medical care provider, our relationship is with YOU, our patient, not with your insurance company. We cannot accept the sole responsibility of managing your claims with your insurance companies or any other persons. While the filing of your insurance claims is a “Courtesy” that we extend to our patients, all charges are your responsibility from the date of the services rendered.
Your health insurance is a contract between you, your employer, and the insurance company. Generation Care is not a party to that contract. We do not have access to the same information that you do. It is very important that you understand the provisions of your policy. We cannot guarantee payment on any claim. If your insurance company pays only a portion of the bill or rejects your claims, any contact or explanation should be made to you, the policyholder. Our priority is your care. We will provide your insurance company with any documentation and medical records they require…at no charge to you.
Our fees are based on national and regional comparative data, and we charge the lowest fees in the area. In addition, we are contracted with many insurance companies, and we are obligated to accept their fee schedule despite the fees set by our clinic. If your insurance company denies your claim we are not obligated to hold to the contracted amount; however, Generation Care will match the fee set by your insurance carrier. The patient is responsible for payment in full within a reasonable time…regardless of the status of the claim with your insurance company.
Even though an insurance claim has been filed, you will receive a monthly statement if your account has a balance due. We cannot accept responsibility for negotiating a settlement on a disputed claim. We realize that temporary financial problems may affect payment on your account in a timely fashion. If you experience such problems it is your responsibility to contact our financial coordinator to make the necessary arrangements for payment. A $25.00 late fee will be added to any account not receiving payment within 30 days of statement. Generation Care, PLC gladly accepts Cash, Check, Visa, and MasterCard for your convenience. Returned checks are subject to a $25.00 service fee and must be resolved before continuing treatment.
Co-pays are due at the time of service.
